What Is Neuromodulation?
Neuromodulation uses small, programmable devices to deliver gentle electrical signals to nerves. Those signals change how pain messages travel to the brain, so the pain you feel is reduced. It is a well-established, FDA-approved approach that has been used for decades and keeps getting better as devices get smaller and smarter.
Advanced Pain Institute of Texas offers three forms of neuromodulation, so your treatment matches the type and location of your pain:
Spinal Cord Stimulation (SCS)
Thin, flexible leads are placed in the epidural space along the spine and connected to a small rechargeable or long-life battery placed under the skin. SCS is most often used for chronic low back and leg pain, failed back surgery syndrome, diabetic neuropathy in the feet, and complex regional pain syndrome. Our physicians have offered newer stimulation waveforms, including Medtronic’s DTM programming, designed to provide relief without the tingling some older systems produced. Learn more about SCS.
Dorsal Root Ganglion (DRG) Stimulation
DRG stimulation targets a specific cluster of nerve cells next to the spine. Because it is so focused, it can help pain in hard-to-reach areas such as the foot, knee, groin or hip — common trouble spots for CRPS and post-surgical nerve pain. Learn more about DRG.
Peripheral Nerve Stimulation (PNS)
PNS places a hair-thin lead next to a single nerve outside the spine — around the shoulder, knee, low back, hip, foot or the back of the head. We offer both a temporary 60-day PNS treatment, after which the lead is removed and many patients keep their relief, and implanted systems such as Nalu for longer-term control. Learn more about PNS.
You Try It Before You Commit
One of the most reassuring things about spinal cord and DRG stimulation is the trial. Temporary leads are placed through a needle — no incision — and connected to an external device you wear for about five to seven days. You go home, follow your normal routine, and judge the results for yourself. Patients who get substantial relief during the trial (typically 50% or more) can move on to the permanent implant; if the trial does not help enough, the leads are simply removed.
What the Permanent Implant Involves
The permanent system is placed during a minimally invasive outpatient procedure through one or two small incisions, and most patients go home the same day. You will avoid heavy lifting, twisting and reaching overhead for several weeks while the leads settle, then gradually return to activity. Afterward, you control your stimulation with a handheld remote or phone app, and our team fine-tunes programming at follow-up visits. Systems can be turned off and removed if you ever no longer need them.

Who Is a Good Candidate?
Neuromodulation is usually considered when:
- You have had back, neck, leg or nerve pain for six months or longer
- Injections, medication and physical therapy no longer provide lasting relief
- You still have pain after spine surgery (failed back surgery syndrome)
- You have CRPS, painful diabetic neuropathy, or nerve pain after injury or surgery
- You are not a candidate for, or want to avoid, another surgery
- You want to reduce or avoid opioid medication
Most insurance plans require a brief psychological evaluation before a trial. It is a routine step, not a judgment about your pain, and our team will coordinate it.
The Process, Step by Step
- Consultation: We review your history, imaging and prior treatments, and talk through which device type suits your pain.
- Pre-authorization and evaluation: We handle insurance approval and arrange any required screening.
- Trial (5–7 days): Temporary leads, no incision. You track your pain and activity at home.
- Decision: If the trial gives you meaningful relief, we schedule the permanent implant.
- Implant and follow-up: Outpatient procedure, then programming visits to fine-tune your settings.
For temporary 60-day PNS, there is no separate trial — the treatment period itself is the therapy.

Frequently Asked Questions About Spinal Cord & Peripheral Nerve Stimulation
The trial leads are placed through a needle with local anesthetic and light sedation, so there is no incision. Most patients feel some soreness at the insertion site for a day or two. You go home the same day.
Spinal cord stimulation places leads along the spine and is best for widespread back and leg pain. Peripheral nerve stimulation places a tiny lead next to a single nerve outside the spine, such as at the shoulder, knee or low back, and is best for pain in one specific area. Your physician will recommend the option that matches your pain pattern.
Many current systems are approved for MRI scans under specific conditions. If you expect to need MRIs in the future, tell your physician during the consultation so we can select an MRI-conditional device.
Medicare and most commercial plans cover spinal cord stimulation for qualifying conditions such as failed back surgery syndrome, CRPS and painful diabetic neuropathy, usually after a successful trial and prior authorization. Our team manages the authorization process. Call (972) 866-4246 or submit a benefit verification request to check your coverage.
Rechargeable systems commonly last many years before the battery needs replacing, and non-rechargeable systems typically last several years depending on your settings. Battery replacement is a short outpatient procedure.
Procedures and device programming are done at our Lewisville office inside Medical City of Lewisville Hospital. Some routine follow-up conversations can be handled through telemedicine. The office is about 25–30 minutes from Frisco and downtown Dallas.
